Minnesota

Assisted Living (24-Hour Customized Living) Services

In Minnesota, ASI is licensed as a Class F Home Care Provider and is a Housing With Services establishment through the Minnesota Department of Health. Assisted Living is a service option available at select locations in Minnesota to help residents achieve their highest level of independence.

Residents with mobility impairments and/or brain injuries or similar cognitive disabilities have access to on-site Resident Assistant Staff 24 hours per day. Services are directed by the resident and may include transfers, help with ADL’s including, medication assistance, housekeeping and meal preparation. Residents with brain injuries or similar cognitive disabilities have access to additional on-site support.

Services are supervised by a nurse who works with the resident to develop an individualized care plan. A Site Supervisor works directly with the resident to coordinate schedules and implement their care plan. Payment available through Home and Community waivers.

Independent Living Services (ILS)

Independent Living Skills (ILS) Services are designed to help adults with brain injury and/or other physical disabilities reach their highest level of independence through personalized training. Participants live in their own homes and work with Independent Living Skills Workers on a regular basis. Specific services are arranged according to the needs of each individual. Applicants must be eligible for Minnesota's Traumatic Brain Injury (TBI) Waiver, the Community Alternatives for Disabled Individuals (CADI) Program, or enrolled with AXIS Healthcare.

Licensed Corporate Adult Foster Care (Shared Living)

Licensed Adult Foster Care "Shared Living" group homes are located in the Twin Cities metro area. Each house is an accessible, affordable home for persons who have a traumatic brain injury or who have similar needs requiring a supportive living environment. Each Resident has a private room and shares the common areas with 3 housemates.

Residents have access to 24-hour, on-site staff. Residents work with a Site Supervisor along with County Case Managers to identify care needs and set personal goals. Staff work with residents to increase independence in physical cares, household cleaning tasks, problem solving, scheduling appointments, transportation arrangements and guidance in behavior management and socialization.

Applicants must be eligible for Minnesota's Medical Assistance Program (Medicaid) and have either the Traumatic Brain Injury (TBI) Waiver or Community Alternatives for Disabled Individuals (CADI) Waiver.

Senior Assisted Living Services

Kenosha Drive Apartments, Rochester, MN, provides 53 units of affordable and accessible housing to Seniors age 62 or better. ASI offers 24-Hour Assisted Living/Customized Living Services to the Residents of Kenosha Drive Apartments.

Qualifying residents at Kenosha Drive Apartments who have a service need (including physical disabilities or brain injury or similar cognitive disabilities) will have access to on-site Resident Assistant Staff twenty-four hours per day. Service may include assistance with service coordination, activities of daily living, medication set up and administration, housekeeping, laundry, meal preparation, essential shopping, activities and behavior management. Residents with brain injuries or similar cognitive disabilities have access to additional supportive services.

Residents choosing Assisted Living/Customized Living Services enjoy increased independence and greater flexibility utilizing staff availability on a 24/7/365 basis to assist with scheduled and intermittent cares. The services are supervised by a Site Nurse who works with the resident to develop an individualized care plan and coordinated by a Site Supervisor who works with the resident to implement the care plan.

Montana

Montana Personal Care Services

ASI offers personal care services in Great Falls, Montana. Residents have access to on-site Resident Assistant staff 24 hours per day via call system. Services are directed by the resident and may include help with activities of daily living, housekeeping, meal preparation, behavior management, socialization and service coordination. Services are supervised by a site supervisor who works with the residents to develop an individualized plan of care. The Site Supervisor also works with the residents to coordinate staffing schedules and implement the care plans. Resident must be eligible for funding through PAS Plan and for Easer Seal Medicaid Waiver.

Nevada

Nevada Community Enrichment Program (NCEP)

NCEP is a highly recognized leader in the field of brain injury rehabilitation. We are the only non-profit, post acute brain injury rehabilitation program in Nevada serving individuals with acquired and traumatic brain injuries (ABI and TBI), as well as neurological impairments.

NCEP has earned and maintained CARF (Commission on Accreditation for Rehabilitation Facilities) Accreditation for adult, adolescent and pediatric Out-Patient Brain Injury Rehabilitation, Residential Brain Injury Rehabilitation, and Vocational Rehabilitation.

In 2011, NCEP has been selected as a contracted provider with the Veterans Health Administration Assisted Living Pilot Program for Veterans with Traumatic Brain Injury. NCEP’s comprehensive day and residential treatment programs provide specialized, evidence based, client centered rehabilitation services including: comprehensive evaluation and treatment plan, medical management, case management, physical therapy, occupational therapy, speech-language pathology, cognitive retraining, vocational services, behavioral management, counseling, life skills training, and community re-entry.

Basic admission criteria includes: diagnosis of traumatic or acquired brain injury, or other neurological condition; medically stability; ability to voluntarily and actively participate in a minimum of 3 hours of rehabilitation per day.

Funding and payment for services includes: Medicaid, worker’s compensation, private and commercial insurance contracts, private pay agreements, and special grant funding may be available for those who qualify.

Nevada Supportive Housing (NSH)

NSH enables individuals who have a physical disability and/or brain injury to live independently with access to 24 hour per day shared supportive living services. This program is open to Medicaid eligible consumers who live at Carol Haynes Apartments and Sandy Robinson Apartments in Las Vegas, Nevada, and Frost Yasmer Estates in Carson City, Nevada.

Northern Nevada Supportive Living Services (NNSLS)

Northern Nevada Supportive Living Services (NNSLS) provides both the best and the most comprehensive training to both our staff and the individuals we support. Individuals who receive our services will achieve the greatest independence and integration into the communities where they live and work. Whether you need to learn home management and personal hygiene skills, money management, or skills in using public transportation, we're here to coach and direct you toward your greatest independence. NNSLS staff are committed to providing recreational opportunities to the individuals we support. We are involved with the Far West Disabled Sports program, offering annual skiing and snowboarding trips in the Winter and River rafting and 4 wheeling in the Summer. Other activities include weekly bowling groups, trips to Lake Tahoe, Wild Islands, and Six Flags. NNSLS also offers weekly social skills classes, art classes, and cooking classes. We have volunteers who provide assistance at various places within their community.

North Dakota

North Dakota Assisted Living Services

ASI is licensed as an Assisted Living Facility with the North Dakota Departments of Health and Human Services. Those services are available at The Dewey Apartments in Jamestown.

Service residents have access to on-site Resident Assistant staff 24 hours per day via call system. Services are directed by the resident and may include help with activities of daily living, housekeeping, meal preparation, behavior management, socialization and service coordination. Services are supervised by a Site Supervisor who works with the resident to develop an individualized plan of care. The Site Supervisor also works with the residents to coordinate staffing schedules and implement the care plans.

Residents must be eligible for funding through the SPED and/or Medicaid Waivers for Home and Community Based services.
